Down syndrome is a complex condition. Individually, children with Down syndrome can differ markedly from one another in their medical, developmental and behavioral attributes and academic skills. Understanding any particular child's strengths and challenges is critical to successful learning and inclusion both in the classroom and across community settings. Some children have especially complex-needs because of the particular constellation of medical-developmental-behavioral challenges. Daytime neurobehavioral problems may be present in ~20% of children with Down syndrome and frequently includes a component of disturbed night-time sleep. Chronic sleep disturbance, thyroid disease and other medical illnesses can easily undermine attentional control and the regulation of behavior in young children with intellectual disability. Treatment of underlying medical problems, individualized education plans, behavioral management techniques and judicious use of medications to minimize impairing symptoms are all being used with varying degrees of success. Strategies for recognizing, prioritizing and managing children with complex medical-developmental-behavioral needs in the home and school setting will be discussed.
